HIGHLIGHTS (250 words)
The main focus area of the Centre has been on development of a model of comprehensive health services through its urban and rural health programs. The Urban health program provides community based health services to an urban resettlement colony in Dakshinpuri Extension. The Comprehensive Rural Health Services Project (CRHSP), established in Ballabgarh Block, Faridabad District of Haryana is a collaborative project between Haryana State Government and AIIMS. CCM is the nodal department for Disease Prevention and Outbreak Response Cell (DPORC) at AIIMS with the mandate of prevention and control of vector and water-borne diseases in AIIMS campus. CCM houses the WHO Collaborating Centre for Capacity Development and Research in Community based NCDPC. CCM has contributed significantly in providing technical inputs into the National Program for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases including Stroke (NPCDCS), developing program managers training manuals as well as co-ordinating the National NCD monitoring Survey across ten implementing agencies; Has contributed to elimination of IDD in India, Bangladesh and Sri Lanka and National HIV/AIDS surveillance. Departmental projects with CDC Atlanta has helped in elucidating the epidemiology of acute respiratory infections and viral pathogens in India.
CRHSP Ballabgarh (Sub district hospital and two PHCs) provided the following patient care services:
Under Urban Health Program (UHP), a total of 24,617 patients were seen including immunization.
EDUCATION
Undergraduate: MBBS students in their 4th-5th semester are posted in the urban health program for five weeks and conduct epidemiological exercise, health talks and family studies. During these two semesters, they also participate in the Family Health Advisory Services (FHAS) in which they follow up their allotted families over a period of nine months and implement interventions to improve the health of the family. From January 2017, FHAS was remodeled to focus on chronic diseases and children only. In the 7th Semester, they are posted for six weeks at Ballabgarh. This is a residential posting that includes training in epidemiology, orientation to community health, community aspects of child health and mental health, orientation to district level health systems, and national health programs including special emphasis on IMNCI.
Interns: The interns are posted for a total duration of three months at the CRHSP Ballabgarh. This period is divided into 6 weeks at sub district hospital (SDH), and 3 weeks each at the Primary Health Centres (PHCs) located at Dayalpur and Chhainsa. The posting at PHC is also residential. At SDH, interns are posted in OPD and Wards that help them in learning basic clinical skills, as well as in-patient management, including conducting normal deliveries. They are rotated in all the available clinical disciplines at SDH under the supervision of respective Senior Resident (SR). During PHC posting, interns participate in health care delivery at the clinics of PHC, sub-centres as well as in antenatal clinics. They are also posted at the delivery hut at each PHC under overall supervision of SR of Community Medicine. They get experience in community level activities such as supervision of health workers, follow-up of patients that have defaulted while on anti-tuberculosis treatment, supervision of sub-centres, undertaking outreach activities, immunization and other ongoing activities at the PHC. At the end of the CRHSP posting interns submit their log-book duly signed by respective SRs under whom they were posted for end posting assessment.
Postgraduates of Community Medicine: The postgraduate students of Community Medicine are posted at urban and rural health program for 16 months each. The regular teaching programme includes seminars, family and clinical case presentations, problem based exercises on hospital and field-practice area as well as journal clubs.
B.Sc. Nursing and Post Certificate Nursing Students: B. Sc Nursing students and post certificate nursing students are posted both with urban health program and one and half months at CRHSP. The rural posting is residential. They have to spend one month in SDH and two weeks in PHC Dayalpur. They are provided training in ANC care, ward management and immunization. Training of nursing students is supervised by faculty member from College of Nursing, AIIMS.
In-service Training: In addition to teaching of students, periodic in-service trainings were conducted for different cadres of staff working at the CRHSP Ballabgarh. The trainings conducted during 2016-17 were as follows:
